Job Description The Care Coordinator is the main point of contact for our clients, their family members, and our Caregivers regarding care needs and changes. As the Care Coordinator, you will be responsible for supervising the care of our clients by assessing and monitoring the quality of services provided to them in addition to changes in condition such as illness, injuries, hospitalizations, etc., and managing updates in their Care Plan. This will be accomplished through introductory visits and consistent on-site visits which are an essential role in the Care Coordinator position. The Care Coordinator is also responsible for ensuring all Caregivers are prepared to care for our clients based on their skill level and the clients’ care needs. The Care Coordinator is also responsible for identifying, interacting with, and maintaining key referral sources alongside our Business Development team. They will create a bridge between key referral sources including case managers, discharge planners, administrators and others who work within skilled nursing, senior living communities, hospitals, physician offices, home health and hospice.
